APPLICATION INSTRUCTIONS

PLEASE PRINT OUT THIS FROM AND SAVE IT! YOU WILL BE ABLE TO KEEP TRACK OF YOUR APPLICATION STATUS WITH THIS FORM

Dear Participants and Parents,

Please print out the application forms from the following links. Complete them as soon as you can and then mail them to our office.


Forms Checklist:

__ 4 passport pictures (with name printed on back)

__ Application for Enrollment

__ Health History

__ Medical Examination

__ Parents' Information

__ Statement of Understanding

__ Drug & Alcohol Policy

__ Domestic Travel Info

__ Hotel Request (optional)

__ 2 letters of recommendation

__ 2 copies of first 2 pages of passport

Payment Checklist:

__ $1000 (due 30 days after registration fee was paid)

__ 1/2 trip payment (due April 16, 2008)
